This percussion instrument of prestige is not part of the classic Mwotlap orchestra (na-wha); it is only taken out for na-wha titi performances. The making of na-tmatwoh drums is normally secret, reserved to initiated men. The soft trunk of a sago tree (no-yot) is hollowed out, so as to yield a long cylinder of approx. 2 meters, diameter 40 cm, with sides about 15-mm thick. The lower end of the drum is cut into points, so as to be stuck in the ground. The other end is covered (vil) with a layer of pandanus leaves (na-gavag, nō-yōpdēge) woven into a mat; this forms the membrane that is to be slapped (woh) by the musicians.

Le guérisseur se rend chez le malade pour “tenir son eau” (tēy nē-bē nonon, d'où le nom tēytēy-bē): après avoir versé de l'eau pure dans une coupe (ni-vinlah), il la fait d'abord tourner trois fois autour de la tête du malade. Durant toute la séance, il tient dans l'autre main une plante qui lui est secrète, et qui lui transmet le pouvoir (na-man) de guérir.
Dans un deuxième temps, le guérisseur observe (cf. sēil) cette eau comme une boule de cristal (qele ni-vidio ‘comme la télévision’) : les ombres humaines (ni-nini et) qu'il voit évoluer lui apprennent les causes de la maladie – généralement, des esprits à forme humaine (na-tmat), qui ont assailli la victime dans les jours précédents.
Dans un troisième temps, le guérisseur prend une gorgée de cette eau et la crachote (bus ②) sur le front et le nombril du malade – lequel boit le reste de la potion. Pour finir, il murmure une incantation qu'il a composée dans sa jeunesse, et qu'il est le seul à connaître: il ne dévoilera ses secrets, l'âge venu, qu'au digne successeur qu'il se sera choisi.
